Medicines Reconciliation and Transcription

Course Overview

Aimed at nurses and senior carers, this course will teach you how to reconcile medicines and transcribe medicines safely and accurately onto a medicines chart. This is a practical, assessed workshop. We will design this workshop around the actual medicines charts and other paperwork (e.g. discharge summaries) that you use. As part of this workshop, we can provide you with a medicines reconciliation and transcription policy and medicines reconciliation tool/assessment if needed.

Nurses and senior health and carer professionals working in any health or social care setting (e.g. care homes, supported-living, hospices, community settings)

Learning Outcomes

  • Which sources of information can be used to reconcile a person’s medicines?
  • What checks are needed when those sources disagree with each other
  • What additional checks do NICE guidelines require for medicines reconciliation?
  • Timescales for passing a person’s medicines “list” to other care providers
  • Transcribing medicines onto your own organisation’s medicines charts
  • Transcribing complex dosages (e.g. increasing, reducing, alternate day, weekly doses)
  • Remote prescribing (verbal orders)
  • Prioritizing time-critical medicines
  • Risk assessing higher risk medicines (e.g. NSAIDs, anti-coagulants)
